  The Department of Transportation (DOT) physical examination is a health examination mandated by the  Federal Motor Carrier Safety Administration (FMCSA) for commercial motor vehicle (CMV) drivers. A DOT physical helps determine if a driver is physically, mentally, and emotionally fit to operate a Commercial Motor Vehicle safely. The DOT examination must be performed by an FMCSA Certified Medical Examiner.

1. You are required to complete and pass a DOT physical if you:
2. Operate a motor vehicle for interstate commerce with a gross combination weight rating, gross vehicle weight rating, or gross combination weight of over 10,000 pounds
3. Operate a motor vehicle designed to carry more than 15 people
4. Are paid to operate a motor vehicle designed to carry more than eight people 
5. Transport hazardous materials that required your vehicle to be placarded (carrying over a certain amount of dangerous goods)  

1.  Your government-issued picture ID such as a Driver's license 
2. All your prescriptions or medication bottle(s) if you are taking any medication 
3. Bring your glasses, contact lenses, or hearing aids if you wear them for driving
4. Any waiver, exemptions, or Skill Performance Evaluation (SPE) Certification you hold
